Medication Errors

Medication errors are defined by the Royal College of Nursing as 'any preventable event that may cause or lead to an inappropriate medication use or patient harm while in the control of the health care professional, patient or consumer'.

Medication errors can arise out of dispensing errors by the Pharmacist, mislabelling on products, errors in administration, lack of education, lack of supervision, monitoring and use.

The following are the most common types of medication administration errors:-

  • Omission error;
  • Incorrect dosage;
  • Incorrect medication;
  • Incorrect times;
  • Incorrect preparation of a dose;
  • Incorrect administration technique;
  • Expired drug usage.
